Nurse Care Specialist

About the Role:
As a Nurse Care Specialist with ParaMed Home Health Care, you will have the opportunity to make a meaningful difference in the lives of patients in their own homes. This community nursing role involves scheduled visits to provide care and support to patients of all ages, with varying levels of complexity.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques
- Develop and implement care plans, monitoring patient progress and adjusting as needed
- Build rapport and trust with patients, engaging them in their care and promoting independence
- Maintain accurate and up-to-date patient records

Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
