Registered Nurse Practitioner

We are seeking a highly skilled and dedicated Registered Practical Nurse to join our team at West Park Healthcare Centre. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation and complex care programs.
About the Role:As a Registered Practical Nurse, you will be responsible for providing patient-centered care to individuals with a wide range of medical conditions, including lung disease, amputation, stroke, and musculoskeletal trauma. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, provide education and support to patients and families, and contribute to the maintenance of a safe and healthy work environment.
Requirements:- Diploma in Practical Nursing
- Current and valid Certificate of Competence with the College of Nurses of Ontario
- Ability to achieve 80% in the Functional Independent Measure (FIM) certification exam within the probationary period or within the trial period and on recertification exams if applying to work on any of the following rehabilitation units: 3C (Musculoskeletal Rehabilitation), 3D POD 1 (Amputee Rehabilitation), 2C (Neurological Rehabilitation), 1D (TB Rehabilitation) and 5D (Respiratory Rehabilitation)
- Recent related clinical experience preferred
- Ability to perform the essential clinical duties of the job
- Effective communication skills, problem-solving and decision-making and reliable work habits
