Nurse Practitioner Role

The Nurse Practitioner is a key member of the healthcare team, providing high-quality patient-centered care to diverse client populations. This role requires a strong foundation in advanced nursing practice, health management, and health promotion.
Key Responsibilities:- Provide comprehensive health services, including identification and management of chronic stable conditions, prevention of disease and injuries, health promotion and education, rehabilitation, continuity of care, and support services.
- Assess and provide services to clients, families, and communities, utilizing advanced nursing assessment skills and sound knowledge of clinical therapeutics.
- Work collaboratively with clients and other healthcare providers to deliver high-quality patient-centered care.
- Develop and implement education initiatives for healthcare providers and community members, utilizing demonstrated knowledge and experience of theories guiding adult learning principles and health promotion activities.
- Embody a philosophy of patient and family-centered care, demonstrating leadership in clinical excellence and a commitment to providing an exceptional experience for staff and clients.
- Registered with the College of Nurses of Ontario as an Extended Class RN and a member in good standing.
- Minimum of 5 years of recent clinical experience in Medicine required.
- Membership in the Nurse Practitioner Association of Ontario preferred.
- National certification in Medical/Surgical or Gerontology Nursing from the Canadian Nurses Association required.
- Experience in Case Management.
- Advanced clinical knowledge of current best practices in caring for medicine patients.
- Excellent organizational, critical appraisal, and analytical skills.
- Ability to work independently and as part of a team.
- High level of initiative and self-direction.
- Exemplary interpersonal and communications skills.
- Computer literacy in a Windows environment utilizing Word, Excel, PowerPoint, and Outlook at a senior level.
