As a key member of our primary care team, you will provide comprehensive health care services to our clients, working collaboratively to address the social determinants of health and improve access to care.
Responsibilities:- Provide comprehensive primary health care services, including assessments, diagnosis, treatment plans, and follow-up care.
- Maintain accurate client records and coordinate care within our organization and with other healthcare agencies.
- Determine the need for and order diagnostic tests, interpret results, and counsel clients on abnormal findings.
- Prescribe designated medications and participate in quality improvement initiatives.
- Implement health teaching and promotion with clients, participate in health promotion and community development initiatives, and develop and deliver group programs or presentations.
- Participate in organizational committees, working groups, and team meetings, and consult with team physicians as needed.
- Registered Nurse with the College of Nurses of Ontario in the Extended Class.
- Baccalaureate in Nursing and graduate of a recognized educational program for primary care nurse practitioners.
- 2-3 years of experience in primary care or community nursing, with a demonstrated ability to work with marginalized communities and complex clients.
- Commitment to anti-racism and anti-oppression practices, excellent communication skills, and ability to work flexible hours, including evenings and weekends.
