Job Title: Nurse Practitioner

Mackenzie Health is seeking a highly skilled and experienced Nurse Practitioner to join our team. As a Nurse Practitioner, you will provide high-quality patient-centered care to clients with complex medical needs.

Key Responsibilities:
  • Provide comprehensive health services, including identification and management of chronic stable conditions, prevention of disease and injuries, health promotion and education, rehabilitation, continuity of care, and support services.
  • Assess and provide services to clients, families, and communities.
  • Develop and implement education initiatives for healthcare providers and community members.
  • Collaborate with clients and other healthcare providers to provide high-quality patient-centered care.
Requirements:
  • Registered with the College of Nurses of Ontario as an Extended Class RN and a member in good standing.
  • Minimum of 5 years of recent clinical experience in Medicine required.
  • Membership in the Nurse Practitioner Association of Ontario preferred.
  • National certification in Medical/Surgical or Gerontology Nursing from the Canadian Nurses Association required.
Preferred Qualifications:
  • Advanced clinical knowledge of current best practices in caring for medicine patients.
  • Demonstrated knowledge and experience of theories guiding adult learning principles and principles guiding health promotion activities.
  • Experience in Case Management preferred.
