We are seeking a highly skilled Pharmacist to join our team at Nova Scotia Health and IWK Health. As a Primary Care Pharmacist, you will play a vital role in providing patient-centered care that optimizes safe, effective, and appropriate drug therapy.
Under the supervision of the Manager of Primary Health Care, you will work collaboratively with an interprofessional group of healthcare providers to provide patient-centered care. Your responsibilities will include:
- Working with collaborative family practice teams and primary health care chronic disease management teams to provide patient-centered care
- Identifying and resolving drug-related problems
- Completing medication reconciliation
- Participating in interprofessional medical rounds
- Providing patient education
- Making therapeutic recommendations to the team
You will also be responsible for ensuring safe and effective drug therapy by participating in and overseeing drug distribution systems (inpatient and outpatient) and contributing to the effective and efficient attainment of the goals and objectives of Nova Scotia Health Pharmacy Services.
About YouWe are looking for a Pharmacist with:
- Graduation from an accredited university program in Pharmacy
- Completion of a Pharmacy Residency Program preferred
- Licensed to practice Pharmacy in the Province of Nova Scotia
- Minimum two years' experience in clinical practice in a hospital pharmacy setting preferred
- Strong knowledge of drugs and therapeutics
- Effective oral and written communication skills
- Ability to make accurate and timely recommendations regarding safe and cost-effective use of drugs
- Competencies in other languages an asset, French preferred

Hours- Permanent full-time position; 75 hours bi-weekly
$53.24 Hourly
