About the Role

We are seeking a highly skilled Pharmacist to join our Primary Health Care team. As a Clinical Pharmacist, you will work collaboratively with an interprofessional group of healthcare providers to provide patient-centered care that optimizes safe, effective, and appropriate drug therapy.

Key Responsibilities

  • Work with collaborative family practice teams and primary health care chronic disease management teams to provide patient-centered care
  • Identify and resolve drug-related problems, complete medication reconciliation, and participate in interprofessional medical rounds
  • Provide patient education and make therapeutic recommendations to the team
  • Ensure safe and effective drug therapy by participating in and overseeing drug distribution systems

Requirements

  • Graduation from an accredited university program in Pharmacy
  • Completion of a Pharmacy Residency Program preferred
  • Licensed to practice Pharmacy in the Province of Nova Scotia
  • Minimum two years' experience in clinical practice in a hospital pharmacy setting preferred
  • Strong knowledge of drugs and therapeutics
  • Effective oral and written communication skills
  • Ability to make accurate and timely recommendations regarding safe and cost-effective use of drugs
