Job Description:

We are seeking an experienced Care Manager - Home and Community Care Professional to lead our team in Nova Scotia. As a key member of our management team, you will be responsible for ensuring the provision of safe, effective quality client care within your program area.

The successful candidate will have a minimum of 2 years' experience managing others and will possess excellent leadership, negotiation, conflict management, financial management, and public relations skills. You will also have experience working in a unionized environment and proficiency in using Microsoft Office software.

In this role, you will:

  • Provide professional and practice leadership to staff, ensuring the delivery of high-quality client care services.
  • Monitor performance related to funder expectations, contributing to the development and implementation of quality improvement activities.
  • Assist staff with completing risk assessments, investigations, and reporting related to client or employee incidents.
  • Participate in budget preparation, financial performance management, and measurement reports.
  • Liaise with various members of the home care and clinical management teams, as well as external groups such as LHIN, CCAC, District Health Authorities, Ministries of Health, and community health, social, and service agencies.

You will receive a salary of approximately $83,000 - $100,000 per year, depending on experience, and enjoy a range of benefits, including a defined benefit pension plan, comprehensive health and dental coverage, and paid time off.

Requirements:

To be successful in this role, you must:

  • Hold a Bachelor's degree or equivalent in a related field.
  • Be a Regulated Health Professional with certification as a Registered Nurse.
  • Have a minimum of 2 years' experience managing others.
  • Have experience in community work, working with community partners, and engaging with municipal, provincial, and national external groups.
  • Have strong public relations and networking skills.
  • Be proficient in using Microsoft Office software.
