We are seeking an experienced Pharmaceutical Care Specialist to join our team at Lawtons Pharm in Shelburne, Nova Scotia.
This is a full-time position requiring a high level of patient care and customer service skills. The successful candidate will have excellent communication and interpersonal skills, be able to work independently in a fast-paced environment, and demonstrate a strong commitment to providing exceptional patient-centered care.
The ideal candidate will have a Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution, be licensed as a pharmacist in the province of responsibility, and possess authorization to administer injections.
In this role, you will provide clinical services such as chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, and other prescribing services as appropriate to practicing top of scope in your province.
You will also initiate care plans, follow up, and complete associated documentation, collect relevant patient, drug, and medical condition information, identify and resolve actual and potential drug-related problems, monitor and optimize medication adherence, and provide patient counseling tailored to the needs and concerns of the patient.
Additionally, you will provide superior customer service by acknowledging patients promptly and professionally with warmth, empathy, and compassion.
A competitive total compensation package is offered, including a salary range of $85,000 - $115,000 per year, depending on experience and qualifications, plus flexible work-life arrangements to meet your lifestyle needs.
