Job Summary:

This role involves providing non-medical personal support services, life-skills support, and light housekeeping duties to individuals in the transition from hospital to the community. The successful candidate will work in accordance with the client's service plan, laws, and regulations governing service provision.

Key Responsibilities:

  • Provide personal support services, including assistance with daily living activities, mobility, and communication.
  • Support clients with life skills, such as meal preparation, medication management, and personal care.
  • Perform light housekeeping duties, including cleaning, laundry, and organization.
  • Work collaboratively with clients, family members, colleagues, volunteers, and community partners to ensure high-quality care.
  • Develop and maintain supportive relationships with clients and their families.
  • Communicate effectively with clients, colleagues, and other stakeholders to ensure seamless care delivery.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements:

  • High School Diploma and experience in providing personal support services to individuals with disabilities and/or seniors.
  • Additional certification in personal support care, first aid, health care, or related qualifications preferred.
  • Ability to empathize with individuals with physical and emotional needs.
  • Knowledge of basic math and ability to perform calculations and quantity measurements.
  • Ability to plan and organize work, prioritize tasks, and make sound decisions.
  • Good oral and written communication skills, with a competent level of English.
  • Able to communicate in other languages is an asset.
  • Must demonstrate high commitment to providing exceptional customer service and be reliable, punctual, and capable of managing time effectively.
