The Hope to Health Clinic, Research and Innovation Centre in Downtown Eastside is a unique and innovative health clinic providing primary and specialty care for patients in the Downtown Eastside. The clinic is staffed by an interdisciplinary team and provides "wrap-around" healthcare to clients, ensuring they receive the full continuum of care under one roof.

The BC Centre for Excellence in HIV/AIDS is a world-renowned HIV/AIDS Centre with an innovative, low-barrier approach to healthcare delivery in clinical practice and integrated research concentrations. A multidisciplinary team of clinicians and researchers work collaboratively to improve the health of British Columbians with HIV and communities facing socio-economic barriers in accessing healthcare.

Responsibilities

  • Provide safe and respectful nursing care to an assigned group of patients, according to established policies and procedures, in combination with evolving individualized care plans and setting care priorities.
  • Conduct head-to-toe assessments, monitor and recognize abnormal signs or symptoms, and communicate with the RN/RPN as required.
  • Communicate with physicians regarding assigned patients and carry out verbal and/or written orders as required.
  • Document care provided, including assessments, status, progress, treatments, changes in condition, medications administered, and discharge planning.
  • Prepare patients for and/or receive patients from surgical and/or diagnostic procedures, transfers, and admissions.
  • Administer medications, including narcotics, according to established procedures, and monitor medication effects.
  • Use evidence-based resources to keep nursing knowledge and procedures up-to-date.
  • Respond to incidents such as Codes, injuries, or falls, hazardous spills, aggressive behavior, and fire alarms.
  • Assist with the orientation of new staff and student preceptors.
  • Identify patient and family learning needs and provide educational and reference materials.
  • Participate in care planning meetings and family conferences.
  • Use knowledge of patient status, external supports, and other relevant factors to assist with discharge planning.
  • Participate in staff meetings, special projects, and relevant hospital committees.

Requirements

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Current full practicing licensure with the British Columbia College of Nurses and Midwives.
  • Current basic cardiac life support certification.
  • One year recent, related experience working in an Urban Health setting, including experience with addiction, HIV/AIDS, or Indigenous patient populations.
