Job Summary
Vancouver Coastal Health is seeking a Licensed Practical Nurse to join the Integrated Care Team serving the Downtown Eastside Community in Vancouver, BC. As a member of this team, you will work collaboratively across multiple sites, clinics, and services to provide holistic care to primary care clients.
Key Responsibilities
- Provide comprehensive care to clients and families experiencing health challenges across the primary care continuum.
- Work closely with an interdisciplinary team to assess, plan, implement, evaluate, and document client care.
- Establish and maintain therapeutic relationships based on respect and empathy.
- Support clients and their families to achieve and maintain optimal health.
Qualifications
Education & Experience
- Graduation from a recognized program for Practical Nurses and current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year of recent related experience or completion of a full final clinical placement in a community setting, working as a Licensed Practical Nurse providing care through a holistic mental health, substance use, and/or primary care lens.
Knowledge, Skills & Abilities
- Comprehensive knowledge of other health care disciplines and their role in client care.
- Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity, and professional judgment.
- Ability to assess clients at risk and knowledge of appropriate acts; Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.
