Licensed Practical Nurse, BC-CfE Hope to Health Primary Care

Job Summary:
The Hope to Health Clinic, Research and Innovation Centre in Downtown Eastside (DTSE) is a unique and innovative health clinic providing primary and specialty care for patients/clients in the DTSE. As a Licensed Practical Nurse (LPN), you will be part of an interdisciplinary team focused on delivering high-quality care to our patients/clients.
Key Responsibilities:
- Provide safe and respectful nursing care to an assigned group of patients/residents, according to established policies and procedures.
- Conduct head-to-toe assessments, monitor and recognize abnormal signs or symptoms, and communicate with the RN/RPN as required.
- Communicate with physicians regarding assigned patients/residents and carry out verbal and/or written orders as required.
- Document care provided, including assessments, status, progress, treatments, changes in condition, medications administered, and discharge planning.
- Administer medications, including narcotics and excluding intravenous route, according to established procedures.
Requirements:
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
- Current full practicing licensure with the British Columbia College of Nurses and Midwives.
- Current basic cardiac life support certification.
- One year recent, related experience working in an Urban Health setting, including experience with addiction, HIV/AIDS, or Indigenous patient populations.
