Key Responsibilities:
- Provide individualized rehabilitation programs to clients with ABIs, promoting independence and increasing levels of functioning.
- Assist clients with personal care, medication management, and transfer tasks, ensuring their safety and well-being.
- Utilize crisis and behavioral management strategies to support clients with challenging behaviors.
- Document client progress and maintain accurate records, adhering to company and provincial guidelines.
- Collaborate with the rehabilitation team to develop and implement individualized plans, ensuring seamless care coordination.
- Participate in ongoing education and training to enhance skills and knowledge in rehabilitation and support work.
Requirements:
- Post-secondary education in social sciences or health care related studies.
- PSW certification and experience preferred.
- Valid G driver's license, access to a vehicle, and proper insurance for transporting clients.
- Ability to work flexible hours, including evenings, weekends, and occasional holidays.
