Community Health Navigator, Correctional Services Specialist, MSW

Job Summary
As a Community Health Navigator, you will play a vital role in coordinating referrals to care, working with Indigenous partners, and facilitating holistic care planning for clients with complex needs.
Key Responsibilities
- Coordinate referrals to primary care, social supports, and other services, ensuring culturally informed and client-centered care.
- Work with Indigenous partners and the FNHA to provide culturally safe and trauma-informed services.
- Facilitate care planning, engaging with service providers and leveraging existing resources to support client outcomes.
- Provide brief supportive counseling and information services to clients, establishing trust and credibility.
- Network with service providers and build collaborative relationships to support care planning and coordination.
Requirements
- Master's Degree in Social Work or Behavioural Science from a recognized program.
- Minimum one year of recent related clinical experience working with complex care individuals with significant mental health, substance use issues, and/or cognitive limitations.
- Demonstrated systems thinking skills, knowledge of community development, and commitment to strengths-based, client-centered practice.
- Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action, In Plain Sight Report, and other related reports.
