Coquitlam, British Columbia, Canada PHSA Full time
Care Coordination and Community Engagement

As a Clinical Care Coordinator with Correctional Health Services, you will play a vital role in coordinating referrals to care, particularly primary care and social supports, from a culturally informed, client-centered, client-choice based care model. You will work with Indigenous partners and the FNHA in partnership, prioritizing the best interests of clients and families. In cases where clients have complex or unique needs, you will facilitate holistic care planning, proactively engaging with and coordinating inputs from a range of different service providers, influencing health and community care providers, and leveraging existing service providers to bridge health and social offerings, ultimately supporting the best outcomes for clients. Referrals may support access to counseling services, income assistance, housing, legal support, food, education, immigration support, employment training, disability assistance, clinical case management, mental health and addictions services.

Collaboration and Community Building

You will participate in care coordination meetings and multidisciplinary case conferences as needed to support client access to care. You will network with service providers and build collaborative relationships to support care planning. You will work alongside community members, community service providers, and health service professionals to engage in community capacity building efforts, focusing on supporting and expanding local service options and enhancing services.

Client Support and Information

You will provide brief supportive counseling and information services to clients, establishing trust, confidentiality, and credibility with community members. You will review referral information and case histories, interview service providers, referral agents, and other sources, determining risk factors for threats of harm to self and others, mental health issues, and substance misuse. You will maintain knowledge of various care pathways, service options, and care providers across the province to facilitate accurate information provision and referrals.

What We Offer

As a Clinical Care Coordinator with PHSA, you will enjoy a comprehensive benefits package, including a municipal pension plan, and psychological health and safety programs and holistic wellness resources. You will have access to professional development opportunities through our in-house training programs, including +2,000 courses, such as our San'yas Indigenous Cultural Safety Training course, or Core Linx for Leadership roles. You will also enjoy annual statutory holidays (13) with generous vacation entitlement and accrual. PHSA is a remote work-friendly employer, welcoming flexible work options to support our people (eligibility may vary, depending on position). You will have access to WorkPerks, a premium discount program offering a wide range of local and national discounts on electronics, entertainment, dining, travel, wellness, apparel, and more.
