Job Title: Indigenous Health Navigator



At PHSA, we are seeking a skilled Indigenous Health Navigator to support our patients in accessing culturally safe care. As a key member of our team, you will work collaboratively with healthcare providers, communities, and organizations to promote health equity and reduce health inequities.



Key Responsibilities


  • Provide consultative and navigational services to patients referred to PHSA programs and services, supporting cultural interpretation and/or health literacy translation services.
  • Support patients in communications with physicians, nurses, and other frontline care staff, ensuring that their cultural and linguistic needs are met.
  • Develop and maintain relationships with Indigenous communities and organizations, promoting health equity and reducing health inequities.
  • Support access and coordination of services for patients within the designated region, ensuring that they receive culturally safe care.
  • Advocate for the needs of patients within the healthcare system, liaising with other community and regional health organizations to improve patient care.
  • Provide support and act as a resource for healthcare providers and other staff members, regarding the provision of culturally safe care.
  • Maintain and update appropriate documentation and records related to the needs of Indigenous patients in the healthcare system.


Required Qualifications


  • A level of education, training, and experience equivalent to a diploma or degree in a related health discipline, health or social sciences, or a patient care/clinical related discipline.
  • One to three years' recent related experience working in Indigenous healthcare, with a strong understanding of Indigenous specific racism and its impact on health outcomes.
  • Ability to travel, with a valid driver's license.
