About the Role

We are seeking a skilled Care Coordinator to join our team in Correctional Health Services at PHSA. This is an exciting opportunity to make a difference in the lives of individuals with complex mental health and substance use challenges.

Job Summary

The Care Coordinator will work closely with clients, healthcare providers, and other stakeholders to provide comprehensive care coordination services. This includes assessing client needs, developing individualized care plans, and coordinating access to community resources and healthcare services.

Responsibilities
  • Establish therapeutic relationships with clients and their families
  • Conduct thorough assessments to identify client needs and priorities
  • Develop and implement individualized care plans that address client goals and outcomes
  • Coordinate access to community resources and healthcare services, including referrals to specialist clinics and treatment programs
  • Monitor client progress and adjust care plans as needed
Requirements
  • Bachelor's degree in a related field (e.g., nursing, social work, psychology)
  • Minimum 1 year of experience working in a healthcare or social services setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of local community resources and healthcare services
