Job Title: Registered Practical Nurse - Primary Care

We are seeking a highly skilled Registered Practical Nurse to join our Interprofessional Primary Care team at the Hamilton Family Health Team. As a key member of our team, you will play a vital role in supporting patient attachment to primary care, promoting attendance at groups, and organizing community events.

Responsibilities:
  • Support patients to access primary care, ensuring warm transitions and collaboration with other community partners.
  • Maintain statistics for attachment of patients.
  • Promote groups and workshops to patients and partners.
  • Participate in community events to support individuals in finding family doctors and promoting attendance at HFHT groups and workshops.
  • Responsible for TB clinic for HFHT patients, including testing and assessment.
  • Support primary care at Eva Rothwell or other clinics as needed.
Requirements:
  • Registered Practical Nurse diploma.
  • Certificate of registration from the College of Nurses of Ontario.
  • Three to five years of nursing experience in a community setting or a combination of community, hospital, or public health settings.
  • Experience in primary care is an asset.
