Senior Palliative Care Nurse

We are seeking an experienced Senior Palliative Care Nurse to lead our new palliative care service in South East Queensland. This is a unique opportunity to be part of a multidisciplinary team that provides leadership in the development, implementation, and evaluation of plans, programs, and models for improving Aboriginal and Torres Strait Islander health and wellbeing.
As a key member of our team, you will work collaboratively with other healthcare professionals and community leaders to create connections and ensure palliative care tools, services, training, and information are developed and delivered in accordance with the Institute for Urban Indigenous Health (IUIH) Model of Care.
You will enable delivery, connection, and partnerships to ensure that resources are used efficiently, avoiding duplication, and building a sustainable and scalable service for our clients and families.
With a focus on quality improvement, the role supports IUIH Member Services in their primary health clinics, as well as providing support for outreach services across South East Queensland.
Key Responsibilities:- Design and deliver the new palliative care service, including collaborating with the project team and Member Services to identify ways to connect to existing programs and services.
- Support the mapping and scoping of the existing workforce skills, capacity, and training needs and options.
- Develop responsive palliative care, end-of-life care, and voluntary assisted dying tools, training, resources, and information to support one-to-one discussions with clients, their families, and with IUIH staff and Member Services.
- Provide leadership in the design and development of clinical services, with responsibilities and oversight of safety and quality of care.
- Provide day-to-day palliative care support in a professional, confidential, and culturally safe manner.
- Undertake comprehensive assessments of clients' health needs in consultation with the client and work with the client and their family to develop a strength-focused plan that sets out their priorities.
To be successful in this role, you will have:
- A current C Class Driver's Licence (QLD)
- APHRA Registration
- A current CPR Certificate
- The ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders, and health professionals, while respecting traditional culture, values, and ways of doing business.
- Demonstrated experience supporting training and development initiatives and resources, with a focus on improving knowledge within a workforce and community.
- High-level communication skills that demonstrate knowledge, clinical reasoning, and the ability to effectively liaise with clients and members of the community, team members, health professionals, and clinic staff in the areas of death and dying and grief and loss.
Estimated Salary: $120,000 - $150,000 per annum, depending on experience.
