Job Summary

The Fraser Health Authority is seeking a skilled Nurse Practitioner to join our team in the Patient Assessment and Transition to Home (PATH) unit. As a Nurse Practitioner, you will assume the role of the Most Responsible Provider (MRP) for patients, providing comprehensive care and making decisions about treatment and management.

Key Responsibilities
  • Diagnose and treat patients with undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice.
  • Monitor ongoing care, order screening diagnostic investigations, and interpret reports to plan treatment.
  • Establish priorities for management of health, diseases, disorders, and conditions, and provide follow-up treatment.
  • Collaborate with physicians and other healthcare professionals to assess and diagnose patient status, develop and implement treatment plans, and admit and discharge patients to facilities as needed.
  • Prescribe medications within statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
Requirements
  • Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
  • Recent relevant clinical nursing experience.
