Primary Care Coordinator

Providence Health Care is seeking a highly skilled and experienced Clinical Coordinator to join our Primary Care team at Foundry Granville. As a key member of our interdisciplinary team, you will provide first-line supervision, coordinate day-to-day operations, and ensure compliance with Health Canada requirements.
Key Responsibilities:- Supervise and coordinate the use of staff, equipment, and resources to ensure optimal client care.
- Evaluate the provision of direct care to clients and their families, ensuring compliance with established standards and protocols.
- Provide direct health care services to designated clients, including physical assessments, specimen collection, and administration of prescribed treatments and medications.
- Coordinate client flow activities, communicating with healthcare professionals to ensure timely and effective care.
- Act as a resource to staff, consulting on nursing professional practice issues and collaborating with the Community Health Centre Coordinator to problem-solve daily operations and workflow.
- Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN or RPN registrant.
- Three years' recent, related nursing experience in Community Health Nursing, including one year's related leadership/supervisory experience.
- Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate.
- STI Management Certified Practice (or evidence of enrollment) required.
