Job Description

Job Summary:

The Clinical Coordinator, Primary Care, Foundry Granville is a key member of our interdisciplinary team, responsible for providing first-line supervision, coordinating day-to-day operations, and ensuring compliance with Health Canada requirements.

Key Responsibilities:

  • Coordinates client flow activities, reviewing clinic processes and developing protocols to improve client flow.
  • Provides clinical direction and coordination of clients, assisting the Manager with administrative activities to ensure efficient use of client care resources.
  • Liaises with designated team members/staff, supporting interdisciplinary practice, quality improvement initiatives, and interdisciplinary integration of services.
  • Ensures staffing levels match client care needs, performing duties such as scheduling, coordinating staff, equipment, and resources.
  • Evaluates the provision of direct care to clients and their families, ensuring care standards are followed.
  • Provides direct health care services to designated clients, performing physical assessments, collecting specimens, and administering prescribed treatments and medications.
  • Coordinates and participates in quality improvement and risk management activities, evaluating the effect of changes in client care practice and generating recommendations for alternative approaches.

Requirements:

  • Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN or RPN registrant.
  • Three years' recent, related nursing experience in the Community Health Nursing, including one year's related leadership/supervisory experience.
  • Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate.
  • CRNBC Sexually Transmitted Infection (STI) Management certification, or evidence of enrollment.
  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Demonstrated ability to adapt to the changing health system and an understanding of the broad determinants of health.
  • Ability to supervise, provide leadership, and work direction to the interdisciplinary team.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to deal effectively with conflict situations.
