Job Description

Job Summary

The Clinical Coordinator, Primary Care, Foundry Granville is a key member of the interdisciplinary team responsible for providing first-line supervision, coordinating day-to-day operations, and ensuring compliance with Health Canada requirements. This role requires strong clinical direction, coordination of client flow, and administrative activities to ensure efficient and optimal use of client care resources.

Key Responsibilities
  • Ensures staffing levels match client care needs by scheduling and coordinating staff, equipment, and resources.
  • Evaluates direct care to clients and their families, ensuring compliance with established standards, policies, and protocols.
  • Provides direct health care services to designated clients, including physical assessments, specimen collection, and administering prescribed treatments and medications.
  • Ensures clinical prioritization of clients requiring service, allocating staff resources, and communicating with healthcare professionals.
  • Works with the Medical Coordinator and clinic physicians to determine client-physician assignments and facilitate smooth transitions.
  • Monitors and coordinates external and internal client flow, maintaining timely and effective care.
  • Acts as a resource to staff, consulting on nursing professional practice issues and collaborating with the Community Health Centre Coordinator.
  • Provides and/or organizes clinical care, education, and counseling using a chronic disease management approach.
  • Assists the Manager with administrative activities, including budgeting, staff allocation, and evaluating client care products and equipment.
  • Participates in quality improvement and risk management activities, evaluating the effect of changes in client care practice and generating recommendations for alternative approaches.
Requirements
  • Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN or RPN registrant.
  • Three years' recent, related nursing experience in Community Health Nursing, including one year's related leadership/supervisory experience.
  • Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate.
  • CRNBC Sexually Transmitted Infection (STI) Management certification, or evidence of enrollment.
  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Demonstrated ability to adapt to the changing health system and an understanding of the broad determinants of health.
  • Ability to supervise, lead, and work collaboratively as a member of an interdisciplinary team.
