About the Opportunity

The Health Services Lead is a key member of the Primary Health Care leadership team, responsible for advancing priorities in the portfolio. They support the development and implementation of assigned Primary Health Care, Wellness, Chronic Disease Management, Department of Family Practice programs services and initiatives in the Shelburne County.

The Health Services Lead works closely with interprofessional teams to manage performance standards and operational requirements. They facilitate the development and implementation of consistent Primary Health Care and organizational priorities, respecting the uniqueness and individuality of practices and communities.

The ideal candidate will have a baccalaureate degree from a health discipline or related field, with a minimum of three years of recent and related progressive leadership experience in a health care setting. Experience in Primary Health Care and chronic disease management is considered an asset.

Key Responsibilities

  • Develop and implement Primary Health Care programs and services
  • Manage performance standards and operational requirements
  • Facilitate collaboration among interprofessional teams
  • Develop and maintain strong working relationships with health care staff and external partners

Requirements

  • Baccalaureate degree from a health discipline or related field
  • Minimum of three years of recent and related progressive leadership experience in a health care setting
  • Current registration with professional association (if applicable)
  • Knowledge of evidence-based models of care for chronic disease management
  • Demonstrated knowledge of primary health care, health promotion, health determinants, and dynamics of health needs in community
