We're seeking an experienced Registered Nurse to join our team as a Nurse Advocate for Community Care. This is a unique opportunity to make a meaningful impact in the lives of our clients and their families.

Salary and BenefitsWe offer a competitive salary range of $85,000 - $110,000 per year, depending on experience. In addition to a comprehensive benefits package, you'll also receive opportunities for education bursaries, tuition assistance programs, and professional development.
Job DescriptionAs a Nurse Advocate for Community Care, you'll work closely with clients and their families to provide individualized care and support. You'll be responsible for conducting comprehensive assessments, developing care plans, and implementing evidence-based practices to promote optimal health outcomes.
You'll also have the opportunity to work collaboratively with other healthcare professionals to ensure seamless care transitions and connect clients with community resources.
Required Skills and Qualifications- Current registration with the Canadian Nurses Association (CNA)
- Experience working in community care settings
- Strong assessment and communication skills
- Ability to work independently and as part of a team
