Community Health Advocate

The Community Health Advocate will be responsible for providing comprehensive social work services to clients in their homes. This includes assessing client needs, developing and implementing treatment plans, and collaborating with healthcare professionals to ensure optimal care.
About the Role
The successful candidate will have a Master's or Bachelor's degree in Social Work and be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers. They will also possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities
- Assess client needs and develop individualized treatment plans
- Provide counseling and therapy services to clients and their families
- Collaborate with healthcare professionals to ensure optimal care
- Maintain accurate records and reports
Requirements
- Master's or Bachelor's degree in Social Work
- Active membership in good standing with the Ontario College of Certified Social Workers and Social Service Workers
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and access to a vehicle (preferable)
