Salary Information
We offer a competitive salary package for this role, with an estimated annual salary range of $85,000 to $115,000 based on experience and qualifications. This figure is inclusive of all benefits and perks provided by SE Health.
Job Responsibilities
Your primary responsibilities as a Nurse Practitioner will include:
- Conducting comprehensive client assessments to identify their unique care needs.
- Developing and implementing personalized care plans that prioritize the well-being and comfort of our clients.
- Collaborating with other healthcare professionals to ensure seamless care transitions and coordination.
- Maintaining accurate records and reporting any concerns or issues related to client care.
Required Skills and Qualifications
To be successful in this role, you must possess the following skills and qualifications:
- A valid RN or RPN license.
- A minimum of 2 years of experience in community care nursing.
- Proven expertise in evidence-informed practice and excellence in care delivery.
- An active driver's license and reliable vehicle for travel.
