Salary
The total rewards package includes a competitive salary of $7500 sign on bonus, plus great benefits and a team that supports your goals. The estimated annual salary is $85,000 - $120,000 based on 40 hours per week, depending on experience.
Job Description
As a Nurse Community Care Specialist, you will be part of our team that brings hope and happiness to clients, homes, and communities across Canada. Your responsibilities will include:
- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community
Required Skills and Qualifications
To be successful in this role, you will need:
- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
- A current CPR certificate is required
- A valid driver's license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire
