Job Title: Nurse Practitioner - Geriatric Care Specialist
Bayshore HealthCare is a leading provider of home healthcare services, and we are seeking a skilled Nurse Practitioner to join our team. As a Geriatric Care Specialist, you will play a critical role in delivering high-quality patient care to seniors in our community.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop personalized care plans
- Order and interpret diagnostic tests, prescribe medications, and perform procedures within your legislated scope of practice
- Collaborate with interprofessional teams to ensure timely responses to patient needs and changes in health status
- Provide direct patient care, focusing on health promotion, safety, and function
Qualifications:
- Completion of a recognized Primary Care or Adult Nurse Practitioner program with a minimum of 3 years of geriatric experience
- Able to prescribe controlled substances as authorized by the College of Nurses of Ontario
- Master of Nursing and current Extended Class certification with the College of Nurses of Ontario
- Sound working knowledge of hospital nursing functions, current needs in nursing, healthcare, and geriatric care
