The Nurse Practitioner is responsible for developing, implementing, and evaluating resident care plans, providing direct comprehensive care to residents with specialized and complex needs. Key duties include:
- Conducting comprehensive assessments, integrating resident perspectives, staff perspectives, and nursing and allied health theory and research.
- Diagnosing acute and chronic health conditions, collaborating with physicians as necessary.
- Developing and implementing integrated strategies for illness prevention, health promotion, health maintenance, rehabilitation, and restorative care.
- Monitoring resident therapy, providing effective pharmacological, complementary, and/or counselling interventions.
- Prescribing pharmacological agents and therapeutic measures, performing additional controlled acts as outlined in the College of Nurses Standards of Practice for RN (EC).
- Participating in policy development and evaluation to improve resident care.
- Determining the need for, ordering, and interpreting diagnostic reports, making clinical decisions accordingly.
- Maintaining accurate and timely documentation and reporting systems.
- Consulting with and referring to other health professionals and community services as needed.
- Collaborating with residents, families, and interdisciplinary teams to formulate care plans and monitor effectiveness.
- Promoting safe work practices and ensuring accident prevention and health and safety responsibilities are met.
The ideal candidate will possess:
- Bachelor of Nursing Science (BNSc) degree, with a Master's degree preferred.
- Post-graduate Primary Health Care Nurse Practitioner Certificate.
- Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
- Recent general nursing experience, with a preference for experience in long-term care settings.
- Corporate competencies: Customer Focus, Results Orientation, Integrity, and Teamwork.
The successful candidate will demonstrate:
- Demonstrated knowledge of primary health care issues, nursing process, and trends in long-term care.
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
- Strong leadership and communication skills.
- Organizational, prioritization, and time management skills.
- Ability to work in a fast-paced environment and adapt to change and competing demands.
- Skills in gerontology, with the ability to focus on special care needs with empathy, respect, and a supportive approach.
- Ability to interact with seniors in a caring and respectful manner.
- Able to maintain confidentiality and promote safe work practices.
- General working knowledge of computer software, including Microsoft Office.
