Job Summary:
We are seeking a skilled and experienced Charge Nurse to join our team at Deep River & District Hospital. As a Charge Nurse, you will play a key role in managing patient care plans, coordinating discharge planning, and providing clinical expertise and advice to patients and staff.
Key Responsibilities:
- Manage Patient Care Plans: Develop and implement individualized patient care plans, taking into account the patient's medical history, current condition, and treatment goals.
- Coordinate Discharge Planning: Work with the multidisciplinary team to ensure a smooth transition of care for patients being discharged from the hospital.
- Provide Clinical Expertise: Offer guidance and support to patients and staff on clinical matters, including medication management, wound care, and infection control.
- Lead Clinical Teams: Supervise and mentor nursing staff, providing feedback and coaching to ensure high-quality patient care.
- Collaborate with Multidisciplinary Teams: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Monitor and Evaluate Clinical Practice: Identify areas for improvement and implement changes to enhance patient care and outcomes.
Qualifications:
- Registered Nurse: Current registration with the College of Nurses of Ontario.
- Education: Bachelor of Science in Nursing or equivalent education.
- Certifications: BCLS certification, with preferred certifications in ACLS, CTAS, and rhythm interpretation.
- Experience: 3-5 years of clinical experience in progressive positions.
- Skills: Excellent communication, organizational, and leadership skills, with the ability to work effectively in a fast-paced environment.
