About the Role

As a Resident Care Coordinator, you will provide assistance with system navigation and coordinate patient flow, working closely with staff to meet client needs.

Key Responsibilities
  • Coordinate patient flow and ensure smooth delivery of care services
  • Provide assistance with system navigation and support clients with their needs
  • Collaborate with staff to identify and address client concerns
  • Develop and implement effective solutions to improve client outcomes
Requirements

To be successful in this role, you will require:

  • Minimum of 3 years clinical nursing experience
  • Experience in case management and adult education
  • Strong organizational and facilitation skills
  • Ability to work independently and as part of a team
  • Demonstrated commitment to continuing professional development
Education and Licenses

You must hold a Bachelor of Science in Nursing (BScN) degree and possess a valid Class 5 driver's license. A Basic Cardiac Life Support Certificate (BCLS) is also required.

Competencies and Skills

We are looking for individuals who possess excellent interpersonal and leadership skills, with the ability to communicate effectively and work collaboratively with others. You will also require strong computer skills and the ability to integrate clinical knowledge and experience into practice.

Salary and Benefits

The salary range for this position is $42,040 to $52,440 per year, depending on experience. As a valued member of our team, you will also receive a comprehensive benefits package, including health and dental coverage, pension plan, and generous paid time off.
