Job Summary:

The Regional Manager - Family Care Team will lead the development and integration of the Central Zone Primary Health Care (PHC) Framework, collaborating with stakeholders to establish and manage the Family Care Team in the Lewisporte area. This role will focus on enhancing the tenants of Primary Health Care, including Active Community Engagement, Access & Continuity of Care, Inter-Professional Collaboration, Building Improvement & Transformation Capacity, and Evaluation.

Key Responsibilities:

  • Framework Development: Develop the Central Zone PHC FCT Framework, collaborating with PHC team members and stakeholders to implement goals within the FCT.
  • Collaboration and Leadership: Collaborate with the Director of PHC, Community Nursing & IPAC, Senior Director of PHC & Community Services, and the Medical Director Primary Care to develop a 3- and 5-year action plan towards PHC Reform.
  • Operational Management: Develop operational and clinical policy for Family Care Teams, ensuring delivery of safe and quality clinical services and developing/implementing standards of care.
  • Communication and Stakeholder Engagement: Facilitate delivery of a communication plan with public and key stakeholders in the geographical area, collecting data for analysis, evaluation, and reporting purposes.
  • Community Needs Assessment: Support and analyze community needs assessment in partnership with Manager - PHC & Chronic Disease, ensuring a best practice approach to program development and service delivery.
  • Electronic Record Keeping: Support and maintain electronic record keeping, integration, and panel management.

Requirements:

  • Education: Undergraduate degree in a health-related discipline, system design, project management, interdisciplinary studies, or public health.
  • Experience: A minimum of 5 years within the last 7 of progressive leadership experience with a proven track record of system transformation and re-design, including experience in PHC management, health program planning, community mobilization, and health education.
  • Skills: Exceptional oral and written communication skills, effective time management skills, and the ability to prioritize, dynamic leadership style focused on engaging others, developing coalitions, and achieving results, proven negotiation and conflict resolution skills, and the ability to function effectively in a high-pressure, diverse, and changing environment.

Preferred Qualifications:

  • Graduate Degree: A graduate degree in a health-related discipline.
  • LEAN/Prosci Certification: LEAN/Prosci certification considered an asset.
  • Electronic Health Solutions: Knowledge and/or experience with electronic health solutions.
  • Rural Healthcare Experience: Experience working within rural healthcare service delivery models.
