Job Title: Case Manager
Job Summary:
Vancouver Coastal Health is seeking a skilled Case Manager to join their Home Health Case Management team at the Central Community Centre in North Vancouver, BC. As a Case Manager, you will be responsible for managing a select population of eligible Home Health clients, determining client goals and needs, and assessing client situations to provide effective care and support.
Key Responsibilities:
- Manage a caseload of Home Health clients, providing supportive case management skills to ensure optimal health outcomes.
- Determine client goals and needs, assessing client situations to develop effective care plans.
- Work effectively with community resources to coordinate care and support for clients.
- Provide continuity of care across the continuum, assessing clients, liaising with team members, and coordinating clinical interventions.
- Act as a clinical resource for the interdisciplinary team, providing information on specialized theory and practice related to clients with complex health care needs.
Qualifications:
Education & Experience:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience.
- Valid BC Driver's License.
Knowledge & Abilities:
- Comprehensive knowledge and demonstrated ability to apply case management principles.
- Broad knowledge of case management and practice within a client and family-centred care model for clients with complex health care needs.
- Comprehensive knowledge of BCCNM standards of practice and guidelines for clinical practitioners.
- Comprehensive knowledge related to care of clients needing supportive case management.
- Comprehensive knowledge of evidence-based nursing practice related to care of clients needing supportive case management.
