Nurse Case Manager Position

Job Summary
Vancouver Coastal Health is seeking a Registered Nurse to join the team at Lions Gate Hospital in North Vancouver, BC. The successful candidate will work as a Nurse Case Manager, managing a select population of eligible Assisted Living clients.
Key Responsibilities
- Manage a caseload of clients and determine their goals and needs.
- Assess client situations, work effectively with community resources, and plan for care and support.
- Work within a multidisciplinary team context to provide continuity of care across the continuum.
- Act as a clinical resource for the interdisciplinary team.
- Consult, confer, and collaborate with team members, physicians, and other healthcare professionals.
Qualifications
Education & Experience
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle and a valid BC Driver's License.
Knowledge, Skills & Abilities
- Comprehensive knowledge and demonstrated ability to apply case management principles.
- Broad knowledge of case management and practice within a client and family-centred care model.
- Comprehensive knowledge of BCCNM's standards of practice and guidelines for clinical practitioners.
- Comprehensive knowledge related to care of clients needing assisted living case management.
- Comprehensive knowledge of evidence-based nursing practice related to care of clients needing assisted living case management.
