About the Role:
VON Canada is seeking a skilled Licensed Practical Nurse to join our team in Nova Scotia. As a LPN, you will provide medical care to children, youth, adults, and seniors in their homes and other settings across the province.
Key Responsibilities:
- Deliver high-quality patient care, including diabetes management, dialysis, medication management, palliative care, phlebotomy, and wound care.
- Work collaboratively with healthcare teams to develop and implement individualized care plans.
- Conduct home visits to assess patient needs and provide education on self-care and disease management.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements:
- Current licensure as a Licensed Practical Nurse in Nova Scotia.
- Experience in community health an asset.
- Valid driver's license and access to a reliable vehicle.
- Ability to wear Personal Protective Equipment (PPE).
