About the Role
CarePartners is seeking a qualified Community Support Worker to join our team in the Lindsay area. As a key member of our interdisciplinary care team, you will provide high-quality patient care and support to individuals with complex needs.
Responsibilities
- Assist with daily living activities, including personal care, household tasks, and medication assistance.
- Provide cognitive, social, and emotional support to patients.
- Contribute to patient status reports and participate in care planning meetings.
- Prepare nutritious meals and snacks, adhering to patient care plans and dietary requirements.
Requirements
- Strong desire to make a positive impact on patient lives.
- Exceptional interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Valid driver's license and vehicle insurance.
- Community Support Worker education or equivalent, with a preference for those with a PSW certificate.
- Current vulnerable sector check screening and clear background check.
