Job Summary:
We are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will have the opportunity to work with patients in their homes, providing a range of services including nursing care, education, and support. This is a full-time position with a guaranteed number of visits in Collingwood, ON.
Key Responsibilities:
- Develop and implement effective care plans in collaboration with patients, families, and healthcare teams
- Provide high-quality nursing care and education to patients and their families
- Conduct home visits to assess patient needs and provide care
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Participate in ongoing education and professional development to stay current with best practices and industry developments
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong interpersonal and communication skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
