Community Health Specialist

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse.
Key Responsibilities- Develop and maintain effective care plans for patients in their homes, clinics, or other community settings.
- Provide exceptional nursing services to patients, including in-person and virtual visits.
- Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care.
- Perform other clinical duties as required, including administering medications, wound care, and vital sign monitoring.
- A competitive wage grid that recognizes nursing skillset and experience.
- PAID MILEAGE at $0.45 per/km PLUS travel time.
- Flexible hours, autonomy over your schedule, and a variety of assignments.

- Up-to-date mobile devices with current scheduling and virtual technology.
- Current College of Nurses of Ontario (CNO) registration in good standing.

- Exceptional interpersonal skills.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record, including 2-step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
