The Role:
We are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, retirement homes, and schools. Your primary responsibility will be to develop and maintain effective care plans, working collaboratively with patients, families, and healthcare teams to deliver high-quality care.
Key Responsibilities:
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient
- Provide exceptional nursing services, including in-person and virtual care, to patients in their homes and other settings
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Perform other clinical duties as required, including assessments, treatments, and education
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare teams
- Valid driver's license and vehicle, with regular travel required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
