CarePartners is a leading provider of home health care services, seeking a skilled Community Health Nurse to join our team. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities- Develop and implement effective care plans in collaboration with patients, families, and healthcare teams.
- Provide comprehensive nursing services, including assessments, interventions, and education, to patients in their homes, clinics, and other community settings.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
- Participate in quality improvement initiatives and contribute to the development of best practices in community health nursing.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse patient populations.
- Valid driver's license and reliable vehicle, with regular travel required.
- Excellent English verbal and written communication skills.
- CPR Certification and current immunization record, including 2-step TB test.
