About the Job

In this critical position, you'll lead resident move-in, transfer, and move-out processes, ensuring seamless transitions and warm welcomes. You'll also provide social services counseling, develop individualized social work services, and support advanced care planning and treatment conversations.

  • Provide exceptional care and hospitality to residents and their families.
  • Lead resident move-in, transfer, and move-out processes, ensuring smooth transitions and warm welcomes.
  • Develop and implement individualized social work services and support groups to address resident and family concerns.
  • Support advanced care planning and treatment conversations, including funeral arrangement discussions.
What We're Looking For

To succeed in this role, you'll need:

  • A Social Services Worker registration per provincial legislation.
  • Minimum of three years' experience in healthcare, social services, or a related sector.
  • Knowledge of the Long Term Care Act & Regulations is an asset.
  • Strong customer service and communication skills, with the ability to organize, plan, and delegate responsibilities.
