Job Summary

This role involves providing exceptional nursing services to patients in their homes, clinics, retirement homes, or schools. You will develop and maintain effective care plans, work collaboratively with patients and healthcare teams, and provide personalized support to enhance patient outcomes.

Requirements

To be successful in this role, you must possess:

  • Current College of Nurses of Ontario (CNO) registration
  • Registered Nurse (RN) degree or Registered Practical Nurse (RPN) diploma
  • A strong commitment to patient-centered care
  • Exceptional communication and interpersonal skills
  • A valid driver's license and vehicle for regular travel
