Nurse Care Specialist

About the RoleThis is a full-time position that requires a valid driver's license and vehicle for regular travel. You will work independently or as part of an interdisciplinary care team to develop and maintain effective care plans with respect to community health services.
The ideal candidate will have excellent English verbal and written communication skills, a current immunization record, including a 2-step TB test, and a strong commitment to patient-centered care.
Key Responsibilities- Provide exceptional nursing services to patients in their homes, clinics, retirement homes, or schools
- Develop and maintain effective care plans with respect to community health services, independently or in cooperation with Home and Community Care Support Services (HCCSS)
- Communicate and liaise with patients, family members, and other healthcare professionals as part of the interdisciplinary care team
