CarePartners, a leading provider of home health care services in Ontario, is seeking a highly skilled and dedicated Nurse Specialist - Community Health to join our team.
About the Role:
We are looking for a registered nurse with excellent communication skills and a passion for delivering high-quality patient care. As a Nurse Specialist - Community Health, you will work closely with patients, families, and healthcare teams to provide comprehensive nursing services in various settings, including homes, clinics, retirement homes, and schools.
Responsibilities:
- Develop and implement effective care plans in collaboration with patients, families, and healthcare professionals
- Provide exceptional nursing services to patients, including assessments, treatments, and education
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Work independently and collaboratively as part of a dynamic healthcare team
- Maintain accurate and up-to-date patient records
Requirements:
- Current registration with the College of Nurses of Ontario (CNO)
- Diploma or degree in Nursing from an accredited institution
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively as part of a team
- Valid driver's license and reliable vehicle
- Current immunization record, including two-step TB test
