Physiotherapist - Community Care Specialist

We are seeking a highly skilled and experienced Physiotherapist to join our team as a Full Time Community Physiotherapist. As a key member of our team, you will be responsible for providing high-quality physiotherapy services to patients in their homes and communities.
Key Responsibilities:- Assess patients' physical function and develop personalized treatment plans to maximize their independence and safety in their environment.
- Provide physiotherapy services to patients in their homes, community centers, and other settings as required.
- Collaborate with healthcare professionals, caregivers, and patients to ensure seamless care coordination and effective communication.
- Document patient progress, treatment plans, and outcomes in accordance with our policies and procedures.
- Participate in quality improvement initiatives to enhance patient care and services.
- Graduate of an accredited degree program in Physiotherapy (PT).
- Current registration in good standing with the College of Physiotherapists of Ontario (CPTO).
- At least two years of recent clinical work experience as a directing physiotherapist.
