Care and Be Cared For - This is Your HomeAs a Community Physiotherapist, you will be responsible for providing PT services to Ontario Health atHome eRehab patients, maximizing their function, independence, and safety in their environment. You will work in a collaborative team environment, supported by our comprehensive benefits and professional growth opportunities.
Key Responsibilities:- Respond to patient referrals and administer eRehab services in a Directing Therapist Role
- Assess patient treatment needs and develop treatment plans to increase occupational performance, independence, and safety
- Teach and assign patient care tasks to competent individuals and accept responsibility for delegated tasks
- Reassess patient progress and adjust treatment plans as needed
- Manage service delivery in conjunction with healthcare professionals and patients
- Document assessment findings, goals, treatment plans, and interventions
- Protect patient confidentiality and maintain accurate statistics on patient care
- Graduate of an accredited degree program in Physiotherapy (PT)
- Current Registration in good standing with the College of Physiotherapists of Ontario (CPTO)
- At least two years of recent clinical work experience as a directing physiotherapist
- In-depth knowledge of therapy services and community resources in the HNHB Region
- Good understanding of healthcare professional roles for eRehab services
- Relevant physiotherapy experience in a community/healthcare environment
- Comprehensive knowledge of physiotherapy tools, processes, equipment, and assistive technology
- Knowledge of funding agencies related to physiotherapy recommendations
