About the Opportunity

The Healthcare Social Worker plays a vital role in providing comprehensive patient care services to individuals and families within the Nova Scotia Health Authority. As a member of a multidisciplinary team, this position requires a strong understanding of human behavior, systems theory, and family dynamics.

Key Responsibilities:

  • Conduct psychosocial assessments to identify patient needs and develop personalized care plans
  • Provide adjustment therapy, supportive therapy, and crisis counseling to patients and their families
  • Collaborate with healthcare professionals to ensure seamless patient care and effective communication
  • Develop and maintain relationships with community resources and agencies to support patient needs
  • Participate in program planning, consultation, and education to promote best practices in patient care

Requirements:

  • Master's Degree in Social Work from a recognized university
  • Registered or eligible for registration with the NSCSW
  • 2 years of clinical experience as a clinical Social Worker
  • Knowledge of the fundamentals of human behavior, including systems theory, family dynamics, and theories of behavior change
  • Awareness of local community resources and agencies
  • Competencies in other languages an asset, French preferred
