About the Opportunity

The Health Care Social Worker reports to the Program Manager and as a member of a multi-disciplinary team, provides a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include:

  • Psychosocial assessments
  • Adjustment therapy
  • Supportive therapy
  • Crisis counseling
  • Resource counseling
  • Discharge planning
  • Case management

The Health Care Social Worker is actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.

The Social Worker delivers programming with other members of the Nova Scotia Collaborative Inflammatory Bowel Disease Program (NSCIBD). The Social Worker may be required to provide services throughout the Rehabilitation and Supportive Care Services portfolio.

About You

We're looking for a skilled and compassionate Health Care Social Worker to join our team. If you have:

  • A Master's Degree in Social Work from a recognized university
  • Registration with the NSCSW
  • 2 years of clinical experience as a clinical Social Worker
  • Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics, and theories of behaviour change
  • Awareness of local community resources and agencies
  • A valid Nova Scotia Driver's License and access to a reliable automobile
  • Experience working with patients with complex chronic health conditions
  • Experience co-facilitating education sessions with other multi-disciplinary team members
  • Competencies in other languages

Hours
  • Long assignment, Part-time position; 45 hours bi-weekly
  • Approximately an 18-month assignment
Compensation and Benefits

Healthcare Social Worker (Masters) - $49.05 Hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
